We accept all insurance except for state-sponsored Medical Assistance plans. If you have dental insurance, it is best to contact them before your first appointment to find out if you have orthodontic coverage and any specifics regarding your benefits.

Completing our patient forms online before you visit can save you time. Everything is handled electronically so there are no forms to bring with you. Rather, bring your insurance card (if you have one), a photo ID, and, if you are being referred by your family dentist, bring any documentation provided.

Your first appointment will be a consultation. Photos and x-rays will be taken to determine what type of treatment you need. Then, you will meet with Dr. Sydlewski. She will review the treatment alternatives available to meet your orthodontic needs and the dental team will be able to answer any questions you may have.
